Yes. There is something wrong and she needs someone to help her with some part of her life. Any time someone threatens to hurt themself or others there is definately some thing that needs immediate attention. Don't ignore it.

Always is. Why else would someone threaten to take their own life? As a norm you tend to love your flesh and not long to hurt it.It's usually a side effect of depression. Sometimes even chronic depression. Since she's treatening to before doing it, it's a cry for help. If no one helps then it's justification (in her mind) to do it because no one cares.Usually once one has made up their mind to die they begin to enjoy life as weird as that sounds because they've made a peace with their decision. That's why often when someone has been depressed for a long time then suddenly feeling good you really need to watch them for awhile.But I'd definitely get her help.

It depends, why was she threatening to commit suicide?, if she was just doing it for attention you have to find out why she was seeking attention in such a big way. But generally yes threatening to commit suicide shows that there is an underlying issue with her mental health. Try to ask her why she did it and find out if she thinks discussing it with a councillor would help but don't push her into doing it. At this point in her life she will need as many good friends to help support her as possible and you don't want to risk her isolating herself from you just because you've done something to lose her trust.
