early appendicitis can start out with pain at the umbilicus. if the pain starts to shift to the right lower abdomen he needs to see a doctor as soon as possible.

your brother needs to be evaluated. its possible he is feeling an ulcer, gall stones, or a gallbladder attack. if he is weak and tired to a great degree the sooner he goes the better. some ulcers bleed and thus cause weakness. try a good md or a gastroenterologist.goodluck;)michele
